If your loved one dies while receiving hospice care

1. Contact the hospice nurse

The hospice nurse must complete the required pronouncement and confirm that the person can be released into funeral-provider care.

2. Call Atlantis

Tell us whether the death occurred at home, in a hospice facility, hospital or nursing facility. We coordinate the local transfer after release.

3. Arrange securely online

The authorized family member completes selections, required information and signatures online. Our team is available for questions.

Important distinctions

Expected death is not the same as preplanning

This service is designed for an imminent or recent at-need death. It is not a prepaid funeral plan. If your loved one is currently receiving hospice care and death is expected soon, you may review the process and gather information, but transfer cannot occur until the required pronouncement and release are completed.

When can the price exceed $995? Additional charges apply when the person weighs more than 300 pounds. Additional certified death certificates and optional merchandise are priced separately and disclosed before arrangements are completed.

Do I need to visit an office?

No. Arrangements can be completed securely online, with local staff available by phone for guidance.

Does hospice choose the cremation provider?

No. The family or authorized representative chooses the provider. Atlantis is independent and does not imply an exclusive relationship with any hospice.
